{"id":8515,"date":"2024-05-23T12:33:00","date_gmt":"2024-05-23T12:33:00","guid":{"rendered":"https:\/\/wp26.cmswebdeveloper.com\/asian-news\/understanding-the-role-of-a-social-prescriber-at-your-doctors-surgery-and-how-they-can-help\/"},"modified":"2024-05-23T12:33:00","modified_gmt":"2024-05-23T12:33:00","slug":"understanding-the-role-of-a-social-prescriber-at-your-doctors-surgery-and-how-they-can-help","status":"publish","type":"post","link":"https:\/\/wp26.cmswebdeveloper.com\/asian-news\/understanding-the-role-of-a-social-prescriber-at-your-doctors-surgery-and-how-they-can-help\/","title":{"rendered":"Understanding the role of a \u2018Social Prescriber\u2019 at your doctors surgery, and how they can help"},"content":{"rendered":"\n<figure class=\"wp-block-image size-full\"><img decoding=\"async\" src=\"https:\/\/www.asianexpress.co.uk\/wp-content\/uploads\/2024\/05\/south_asian_patient_doctor_social_prescriber_consultation_AdobeStock_113500576_24-jpeg.webp\" alt=\"\" class=\"wp-image-49849\"\/><\/figure>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\"><strong>A Social Prescriber&#8217;s work includes organising social groups, helping with language translation and interpretation for medical appointments, or providing emotional support to patients.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The team at your local GP practice is made up of a range of health professionals, so you can see the right person for your particular health concerns.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We met two health professionals, Zahida and Ruhela, who explained their roles and how the GP practices they work at are helping to improve patients\u2019 experiences.<\/p>\n\n\n\n<div class=\"wp-block-media-text is-stacked-on-mobile\" style=\"grid-template-columns:20% auto\"><figure class=\"wp-block-media-text__media\"><img decoding=\"async\" src=\"https:\/\/www.asianexpress.co.uk\/wp-content\/uploads\/2024\/05\/zahida-iqbal-mugshot-jpg.webp\" alt=\"\" class=\"wp-image-49850 size-full\"\/><\/figure><div class=\"wp-block-media-text__content\">\n<p class=\"wp-block-paragraph\">Zahida Iqbal is a Social Prescriber at The Bridge Primary Care Network in Rochdale. \u201cI\u2019ve been a Social Prescriber \u2013 or \u2018Link Worker\u2019 \u2013 for four and a half years,&#8221; she says.<\/p>\n<\/div><\/div>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cI support patients to take more control of their health and wellbeing, connecting them with local community services that can help with the problems they may have. For example, some people need help with finances, housing, or employment, or support with things like quitting smoking or losing weight.\u201d<\/p>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\">We might not immediately think of these types of issues as something to go to our doctor about, but most of Zahida\u2019s referrals are from GPs, practice nurses or reception staff at the surgery. Zahida explains how working as a team has made a wider range of services accessible to every patient:<br>\u00a0<br><strong>\u201cIf\u00a0you need to see a GP\u00a0you will\u00a0always be offered an appointment, but\u00a0there\u00a0may be\u00a0other health professionals \u2013 like me &#8211; who can provide more\u00a0appropriate\u00a0support. My appointment slots are longer than a GP\u2019s, so I have time to help patients talk through their issues in more detail.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cA GP can provide medical treatment, whereas my experience enables me to come at it from a social, emotional and practical angle.\u201d<br>&nbsp;<br>Zahida also recognises the importance of health professionals getting to know each person they see, and the joint knowledge they are able to use to help every individual.<br>&nbsp;<br>\u201cWhen we take the time to understand each patient and their needs, we can help them even more. The advantage of working as part of a network and an integrated team is that we can each use our own skills to help, so our support to the patient is as holistic as possible and can make a genuine difference.\u201d<br>&nbsp;<br>You can call the reception team at your GP practice, or fill in an online form on your surgery\u2019s website, and you\u2019ll be asked questions that will help to make sure you\u2019re directed to a health professional who is most appropriate for what you need.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">General practice teams can include physiotherapists, clinical pharmacists, mental health practitioners and paramedics, among others.<\/p>\n\n\n\n<figure class=\"wp-block-image size-large is-resized\"><img decoding=\"async\" src=\"https:\/\/www.asianexpress.co.uk\/wp-content\/uploads\/2024\/05\/Ruheela-Begum-768x1024.webp\" alt=\"\" class=\"wp-image-49848\" style=\"width:424px;height:auto\"\/><figcaption class=\"wp-element-caption\">Ruhela Begum<\/figcaption><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">Ruhela Begum is a Wellbeing Practitioner working in Tameside, and is part of a local team of wellbeing practitioners, nurses, paramedics and mental health nurses.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Ruhela has been in her role for over a year and has spent nearly 20-years working in the local community. With such a range of team members and roles, they can work across the community, giving patients access to many different services in their neighbourhoods.&nbsp;<br>&nbsp;<br>\u201cOur role as a team is to support our local GP practices to provide patients with the best possible healthcare and access to local services.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cMy day-to-day work varies from organising social groups, to helping with language translation and interpretation for medical appointments, or providing emotional support to patients.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cSometimes people don\u2019t put their health at the top of their priority list, and it\u2019s my job to help them do this. I\u2019ve learnt a lot from the people I support, and I feel real pride, passion and gratitude for my work at the end of every day.\u201d<br>&nbsp;<br>Ruhela has seen first-hand how changes to the team at her local GP practices have supported patients to feel more in control of their health, by offering a variety of health professionals who can helps with different types of health concerns.<br>&nbsp;<br>\u201cWe\u2019re here to help you, and we\u2019re all working together to make sure our patients get the care they need.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cOther members of the team feel they can approach me to get a better understanding of cultural needs for our community, for example.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cFrom the moment a patient calls their GP practice, the reception team is listening carefully and asking the right questions to understand what that patient needs.\u201d \u00a0<br>\u00a0<br><strong>Your general practice team is here to help you.
